Managing patient in the Last Phase of Life - November 2017
The 30 November saw a gathering of 140 health professionals to hear a wide range of specialists talk about challenges to Palliative care and working to understand how GPs, district nurses, secondary care specialists and the Ambulance Service can work together to develop a coordinated approach. Speakers included Dr Penelope Wiseman, Consultant in Geriatric Medicine from Royal Free NHS Foundation Trust, Dr Caroline Stirling, Consultant in Palliative Medicine and End of Life Care Lead for UCL partner Mr Gerard Bowden of Coordinate my Care and Dr Joanne Brady, consultant in Palliatve Care, North London Hospice.
GPs, secondary care consultants, Paramedics, Nurses and representatives from care homes and voluntary organisations in Barnet worked together to start the process of developing a co-ordinated approach to helping patients in their last phase of life.
